APPLICATION FORMS
We have TWO application forms.

The first is an "on-line" AUTOMATED application. Fill out the form by tabbing from field to field, print a copy for yourself, and hit the "submit" button to send it directly to CCDAL.

IF YOU DO NOT RECEIVE A "CONFIRM RECEIPT" EMAIL RESPONSE WITHIN 3 DAYS, PLEASE CALL OUR SECRETARY AT 301-434-1086.

Automated Application


The second is in MS Word. You can either print the form from your browser, or download and edit the file via the link below. The following steps should help you complete and submit the form:

  • Click on the MS Word version of the form below.
  • Click on the "File" menu and choose "Save as..."
  • Remember where you saved the file for later completion and e-mail.
  • Set your keyboard to "overwrite" to fill in the blanks.
  • Complete the form on your computer.
  • Save the completed form back to your computer with a different name; i.e., (your last name) app for (dog's name).doc.
  • Email your completed application to ccdal@comcast.net

Application in MS Word

If you simply print out the form, you can fill it in by pen and mail it to:

PLEASE NOTE: An application sent by "snail" mail can take up to two weeks to get reproduced and re-mailed to the members of the Application Review Committee, so the preferred method of receipt is by the automated version or email.
